The reason SO MANY men seem unwilling to proceed beyond "Do you want to hang out some time?" and actually ask a woman out on a date that involves going to a specified place at a specified time...

The women who are asking men to treat them as equals are pretty much not the ones asking men to pay for dates. Unless perhaps the man proposed the date, in which case it's just a blanket rule of etiquette that the person who makes the invitation is offering to pay.
